As we are sure you know, the novel Coronavirus, COVID-19, is a global pandemic, it can be extremely contagious, and it is believed to be spread mainly from person-to-person contact and/or by contact with contaminated surfaces and objects. There is an inherent risk of exposure to COVID-19 in public places, like this Club, when others are present. People can be infected and spread the virus even though they may not show any symptoms. COVID-19 can cause serious and potentially life-threatening illnesses.

Oasis On Essington LLC (the "Club") is following the guidelines from the Centers for Disease Control and Prevention as well as applicable state and local directives to help prevent the spread of the disease. The Club, however, cannot guarantee that you will not become infected with COVID-19 by entering this facility. Therefore, if you choose to enter our Club you may be exposing yourself to, and/or increasing your risk of contracting or spreading, COVID-19.

ASSUMPTION OF RISK: I have read and understand the above warning and the risks associated with entering this Club, interacting with the entertainers, and being near other patrons, even though practicing social distancing. I represent that I am informed about the risks involved, I understand that the Club cannot guarantee my safety from COVID-19, and; recognizing these risks, I nevertheless assume all risks of being exposed to and infected with COVID-19 by entering this facility - fully understanding that such exposure may result in personal injury, illness, permanent disability, and even death. 

WAIVER OF LIABILITY: I HEREBY FOREVER RELEASE, DISCHARGE, AND AGREE BOTH NOT TO SUE AND TO HOLD HARMLESS the Club, as well as all of its current and future  owners, directors, officers, legal managers, agents, employees, representatives, entertainers who perform on its premises, affiliated and related entities, successors, assigns, attorneys and insurers (all collectively the "Releasees"), with regard to any and all claims, suits, and causes of action of any kind or nature whatsoever, whether known or unknown and whether foreseen or unforeseen (all collectively the "Claims"), that may result from, arise out of, or that may be related to, my exposure to COVID-19 while on Club property. This release includes, but is not limited to, Claims based on the actions, omissions, or negligence of the Releasees, and is binding on my heirs and representatives.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS DOCUMENT; I HAVE FREELY, KNOWINGLY AND VOLUNTARILY SIGNED THIS DOCUMENT; AND I ASSUME ALL RISKS AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE.
